Controller to ensure the following core actions have been completed:

Obtain details of :-

Location

Staff involved including their contact details and who they work for

Time of incident

What is alleged to have happened

Are there any injuries or any damage to infrastructure

Dispatch MOM / ODM to site and appoint as investigating officer

If a signaller is or could be implicated, arrange for LOM or on call manager to investigate.

Review voice communications/Nice Inform/ Bridgeway on call voice recorder

Advise On Call Maintenance Manager if their staff are implicated.

Advise FOC if their staff are implicated.

If For Cause screening is requested, Update Optima health log

Ascertain if possession will overrun

Establish who will conduct Level 1 investigation

Record in FMS if any damage to infrastructure.

Update CCIL ,FMS and WhatsApp safety alert
