Controller to ensure the following core actions have been completed:
Location/line/train headcode/signal involved and time
How was it brought to a stand (DRIVER/TPWS/AWS etc.)
Reason signal was danger, distance passed signal?
Loco details/unit number/TOC or FOC
Any allegation against Signalling/Track/Stock?
Ensure RT3189 is completed by signaller, can the train be moved clear to a more convenient place.
Dispatch MOM for Cat A testing / swab tests / photographs if required*
Arrange ''For Cause Screening'' (If Req'd)
Advise LOM and S&T (U007 testing (doesn't have to be immediate)
Ascertain whether driver is 'at risk' from TOC/FOC
Request CCTV/OTDR, establish if driver is authorised to continue
Establish train movements with signal box
Advise NOC, Multi Spad location?
RCM to complete Immediate CAT A SPAD form
Update CCIL/FMS and send a Tyrell message and safety text alert out
